About The Role We’re looking for a compassionate and relationship-driven Care Partner to join our Support at Home team. In this role, you’ll walk alongside older Australians to help them live safely, confidently and independently in their own homes. You’ll take the lead in developing personalised care plans, coordinating services, and ensuring each person receives timely, culturally responsive support that reflects their goals and circumstances. Working closely with clinical colleagues, families and community providers, you’ll play a key role in delivering a seamless, empowering care experience. Responsibilities include: Leading the development and ongoing review of person-centred care plans that reflect each participant’s goals, preferences and wellbeing needs Coordinating care and services across Support at Home pathways, ensuring timely, goal-aligned and proactive support Building strong relationships with participants and their support networks to enable shared decision-making and dignity of risk Supporting participants to understand and manage their Support at Home budgets Working collaboratively with clinical Care Partners, multidisciplinary teams and external providers to ensure integrated, high-quality care Maintaining clear, timely communication with families, carers, GPs and allied health partners Ensuring risk identification, incident response and documentation meet organisational, compliance and aged care quality standards About You You’re someone who genuinely cares about supporting older people to live well at home. You bring a calm, confident and compassionate presence, and you’re able to build trust quickly with clients, families and colleagues. You’re comfortable working in a dynamic, people-centred environment and can balance empathy with sound judgment, proactive decision-making and clear communication. You’ll also bring: Case management or care coordination experience in a community setting Strong relationship-building and communication skills Ability to co-design care plans and support goal-setting Understanding of aged care or community support frameworks Confidence using digital care management or CRM systems Cultural competence and a person-centred approach Relevant qualification in Community Services, Aged Care or similar Relevant Nursing or Allied Health qualification (desirable) Care Partners delivering Restorative Care Management must hold a Registered Nurse or Required clearances (NDIS Worker Screening/Police Check) and COVID-19 vaccination About The Process Apply now and join us on the journey to shaping the future of Australian homecare.
